What Is All-on-4?
All-on-4 is an implant technique that replaces an entire arch of teeth — upper, lower or both — using just four titanium implants and a fixed prosthetic bridge. No removable dentures, no adhesives.
The All-on-4 concept was developed to provide patients with extensive tooth loss a permanent, fixed set of teeth without requiring an implant for every missing tooth. By placing two implants vertically at the front of the jaw and two at an angle towards the back, the technique maximises contact with available bone — often eliminating the need for bone grafting.
At Focus Dental, All-on-4 procedures are carried out by our full-time maxillofacial surgeon using 3D CT-guided planning. Implants are placed with precision, temporary fixed teeth are fitted on the same day, and you leave the clinic with a functional smile — not a removable denture.
After a healing period of 3–6 months, you return for the final prosthetic: a custom-designed, screw-retained bridge produced in our in-house digital lab using premium materials.

Regular implants replace individual missing teeth — one implant per tooth. All-on-4 replaces an entire arch (upper or lower) using just four strategically placed implants that support a full fixed bridge. It's designed for patients who have lost most or all teeth in a jaw, offering a complete restoration without needing 8–12 individual implants.
In many cases, no. The All-on-4 technique was specifically developed to avoid bone grafting. The two posterior implants are placed at an angle to engage deeper, denser bone — even in patients with moderate bone loss. However, severe bone loss may still require additional procedures. Our surgeon assesses this during your CT scan before treatment begins.
The surgery is performed under local anaesthesia, so you won't feel pain during the procedure. Sedation options are also available for anxious patients. Post-operative discomfort is manageable with prescribed medication and typically subsides within a few days. Most patients describe it as less painful than they expected.
The full treatment happens in two visits. Your first trip (3–5 days) covers diagnostics, implant surgery and fitting of temporary fixed teeth. After 3–6 months of healing — during which the implants fuse with your jawbone — you return for your second trip (3–4 days) to receive the final permanent bridge.
With temporary teeth, you should stick to soft foods for the first few weeks while healing occurs. Once your permanent bridge is fitted, you can eat virtually anything — steak, apples, nuts. All-on-4 prosthetics are designed to function like natural teeth, with significantly more bite force than removable dentures.
All-on-4 uses four implants per arch, while All-on-6 uses six for added stability. All-on-6 is often preferred when bone density allows it, particularly for the upper jaw where extra support can be beneficial. Your surgeon will recommend the best option based on your CT scan results. Both deliver excellent long-term outcomes.
The titanium implants themselves are designed to last a lifetime with proper care. The prosthetic bridge on top typically lasts 15–20 years before needing replacement, depending on wear and maintenance. Good oral hygiene — brushing, flossing around the bridge, and regular check-ups — is essential for long-term success.
